ELIGIBILITY:

  • Available for Employee ages 18-74
  • Must be actively at work a minimum of 20 hours per week at usual place of employment
  • Must be part of an eligible class on employer’s application

DEPENDENT ELIGIBILITY:

  • Available for Spouse ages 18-74
  • Dependent Children (unmarried) ages 0-25
  • The date the Employee is eligible for coverage, or
  • The date the Employee acquires an eligible dependent
